Your doctor has recommended that you have an endoscopic procedure.  This website contains all the information you will need to decide whether you would like to proceed.

Once you have learned all about the procedure you will be able to sign a Consent Form.

You can print out the completed form on your computer and bring it with you on the day of the procedure.

There will be opportunities to ask questions when you attend.  You can change your mind at any point before the procedure.


If you are unable to keep your appointment, please notify the department as soon as possible. This will enable the staff to give your appointment to someone else
and they will be able to arrange
another date and time for you.
